Introducción: la salud mental de los estudiantes de medicina ha sido ampliamente investigada, demostrando que forman parte de un grupo vulnerable al desarrollo de trastornos mentales. Objetivo: el objetivo de esta investigación fue determinar la frecuencia de NSSI y sus factores asociados en estudiantes de medicina del Paraguay. Metodología: este fue un estudio descriptivo y transversal. Se lanzó una encuesta online para evaluar depresión, ansiedad y autolesiones, se utilizaron las escalas PHQ-2, GAD-7 y SHQ, respectivamente. Resultados: recibimos respuestas de 330 estudiantes de medicina. De los participantes, el 71,2 % eran mujeres. Se identificó que el 46,4 % de los participantes padecía depresión (PHQ-2 ≥ 3) y el 37,3 % ansiedad (GAD-7 ≥10). La frecuencia de NSSI fue del 27 % (n = 89). Los principales factores asociados a NSSI fueron un diagnóstico previo de un trastorno mental (que aumentó la probabilidad de NSSI en 3,76 veces) y/o una historia de abuso físico o sexual (con un aumento de 3,75 veces). Conclusión: esta investigación encontró la presencia de NSSI en casi 3 de cada 10 de los estudiantes de medicina encuestados. Los principales factores asociados con la conducta autolesiva fueron un diagnóstico previo de un trastorno mental y/o antecedentes de abuso físico o sexual.

ABSTRACT

Este estudo teve como objetivo compreender a autolesão na adolescência e investigar sua relação com o funcionamento familiar em um Centro de Atenção Psicossocial Infantojuvenil por meio de um estudo de caso. A autolesão é caracterizada como um comportamento intencional de ferir o próprio corpo, sem a intenção de morrer, podendo evidenciar o desamparo e o convite ao olhar do outro, diante de sua dor. A pesquisa adotou uma abordagem clínico-qualitativa, com análise e discussão dos conteúdos à luz da Psicanálise de Casal e Família. Os resultados revelaram que a adolescente inscreve no corpo um sofrimento não verbalizado, relacionado à crise cultural e à fragilidade dos suportes simbólicos necessários para o desenvolvimento psíquico. Na família, evidenciou-se falhas das funções continentes do Eu-pele familiar, como a dificuldade em perceber os cortes e compreender as queixas da filha. Conclui-se que a autolesão pode servir como uma defesa contra o sofrimento psíquico, comunicando falhas ambientais. Destaca-se a importância da família no desenvolvimento psíquico durante a adolescência, fornecendo apoio emocional diante das transformações e lutos exigidos ao longo da vida.


The aim of this study was to understand self-injury in adolescence and investigate its relationship with family functioning in a Child and Adolescent Psychosocial Care Center through a case study. Self-injury is characterized as an intentional behavior of hurting one's own body, without the intention of dying, and can show helplessness and an invitation to the gaze of others in the face of their pain. The research adopted a clinical-qualitative approach, analyzing and discussing the content in the light of Couple and Family Psychoanalysis. The results revealed that the teenager inscribes non-verbalized suffering on her body, related to the cultural crisis and the fragility of the symbolic support necessary for psychic development. In the family, there were failures in the continental functions of the family Self-skin, such as the difficulty in perceiving the cuts and understanding the daughter's complaints. We conclude that self-injury can serve as a defense against psychic suffering, communicating environmental failures. The importance of the family in psychic development during adolescence is highlighted, providing emotional support in the face of the transformations and mourning required throughout life.


El objetivo de este estudio fue comprender la autolesión en la adolescencia e investigar su relación con el funcionamiento familiar en un Centro de Atención Psicosocial a Niños y Adolescentes a través de un estudio de caso. La autolesión se caracteriza por ser una conducta intencional de dañar el propio cuerpo, sin intención de morir, y puede ser una evidencia de indefensión y una invitación a la mirada de los demás ante su dolor. La investigación adoptó un abordaje clínico-cualitativo, analizando y discutiendo el contenido a la luz del Psicoanálisis de Pareja y Familia. Los resultados revelaron que la adolescente inscribía en su cuerpo un sufrimiento no verbalizado, relacionado con la crisis cultural y la fragilidad del soporte simbólico necesario para el desarrollo psíquico. En la familia, hubo fallas en las funciones continentales de la Auto-piel familiar, como la dificultad para darse cuenta de los cortes y comprender las quejas de la hija. Concluimos que la autolesión puede servir como defensa contra el sufrimiento psíquico, comunicando fallas ambientales. Se destaca la importancia de la familia en el desarrollo psíquico durante la adolescencia, proporcionando apoyo emocional frente a las transformaciones y duelos necesarios a lo largo de la vida.

Introducción: La investigación sobre la autolesión no suicida permite aumentar la conciencia sobre este fenómeno, posibilita su detección temprana y la implementación de estrategias más eficaces en prevención y tratamiento. Esto conlleva a una reducción del sufrimiento individual, los costos económicos y el impacto en la sociedad en su totalidad. Objetivo: Determinar la frecuencia y factores asociados a autolesión no suicida en una muestra de población adulta de Paraguay. Metodología: Estudio descriptivo de asociación cruzada, de corte transversal y prospectivo. Una encuesta en línea fue difundida a través de redes sociales y aplicaciones de mensajería. Esta incluía preguntas sobre datos sociodemográficos y clínicos. La presencia de autolesiones se determinó a través de la versión en español de la escala Self-Harm Questionnaire. Se utilizó estadística descriptiva para todas las variables. Para buscar asociaciones se utilizó la prueba chi cuadrado. Resultados: Participaron 241 personas (media de edad=32±12 años, 74,7% mujeres). Se encontró una frecuencia de autolesiones de 24,5% (n=59). Introduction: Research on non-suicidal self-injury raises awareness of this phenomenon, enabling the early detection and implementation of more effective strategies in prevention and treatment. This leads to a reduction in individual suffering, economic costs, and impact on society. Objective: To determine the frequency and associated factors of non-suicidal self-injury in a sample of the adult population of Paraguay. Methodology: This was a descriptive, cross-sectional, and prospective study. An online survey was disseminated through social networks and messaging applications. The survey included questions on sociodemographic and clinical data. The presence of self-injury was determined by using the Spanish version of the Self-Harm Questionnaire. Descriptive statistics were used for all the variables. The chi-square test was used to examine associations. Results: There were 241 participants (mean age=32±12 years, 74.7%female). ABSTRACT

Risk behaviors are common in adolescence and demonstrates an association with depressive symptoms. Considering the psychological health implications of this phase in adult life, the aim of this study was to verify associations between depressive symptoms and consumption of alcohol and marijuana, self-injurious behavior, health self-perception, life satisfaction, anxiety symptoms and suicidal ideation in adolescents. A total of 298 adolescents, aged 12 to 14 years (61.1% girls), participated in the study. The measures were the translated Health Behavior in School-aged Children questionnaire (HBSC - BR) and the Children's Depression Inventory (CDI). The chi-square test, student's t test and binomial logistic regression (p<0.05) were used. Depressive symptoms were associated with anxiety symptoms and self-injury. Higher risk for alcohol use and consumption, self-injury, negative health self-perception, anxiety and depression related to females. The findings of this population suggest worrying health outcomes, especially for girl. (AU)


Comportamentos de risco são comuns na adolescência e demonstram associação com sintomas depressivos. Considerando as implicações da saúde psicológica dessa fase na vida adulta, o objetivo deste trabalho foi verificar associações entre sintomas depressivos e consumo de álcool e maconha, comportamento autolesivo, autopercepção de saúde, satisfação com a vida, sintomas ansiosos e ideação suicida em adolescentes. Participaram 298 adolescentes, de 12 a 14 anos (61,1% meninas). Os instrumentos utilizados foram o questionário traduzido Health Behavior in School-aged Children (HBSC - BR) e o Inventário de Depressão Infantil (CDI). Utilizou-se os teste Qui-Quadrado, t de student e regressão logística binomial (p<0,05). Sintomas depressivos associaram-se com sintomas ansiosos e autolesão. Maior risco para uso e consumo de álcool, autolesão, autopercepção de saúde negativa, ansiedade e depressão em relação ao sexo feminino. Os achados desta população sugerem resultados preocupantes de saúde, sobretudo para o sexo feminino. (AU)


Las conductas de riesgo son comunes en la adolescencia y demuestran una asociación con síntomas depresivos. Considerando las implicaciones psicológicas para la salud de esta fase en la vida adulta, el objetivo de este estudio fue verificar asociaciones entre síntomas depresivos y consumo de alcohol y marihuana, autolesión, autopercepción de salud, satisfacción con la vida, ansiedad e ideación suicida en adolescentes. Participaron 298 adolescentes de 12 a 14 años (61,1% niñas). Los instrumentos utilizados fueron el cuestionario traducido Health Behavior in School-aged Children (HBSC - BR) y el Inventario de Depresión Infantil (CDI). Se utilizaron las pruebas de chi-cuadrado, t de Student y regresión logística binomial (p<0,05). Los síntomas depresivos se asociaron con síntomas de ansiedad y autolesiones. Mayor riesgo de uso y consumo de alcohol, autolesiones, autopercepción negativa de la salud, ansiedad y depresión en relación con el sexo femenino. ABSTRACT

Comportamentos autolesivos na adolescência se tornaram um problema de saúde pública mundial. Frequentemente, esses comportamentos compõem o quadro sintomático de doenças psiquiátricas, sendo conceitualizados como estratégias disfuncionais de regulação emocional. Diante disso, este estudo investiga a relação entre comportamentos autolesivos, percepção de suporte familiar e sintomas ansiosos e depressivos em adolescentes da rede estadual de ensino do Recife, assim como estima a prevalência do fenômeno (n=501). Também buscou-se estimar a prevalência do fenômeno e sua relação com variáveis sociodemográficas. Os instrumentos utilizados foram o Protocolo Health Behaviour in School-Aged Children, a Spence Children's Anxiety Scale e o Inventário de Depressão Infantil, todos adaptados ao português brasileiro. Foram realizadas análises da consistência interna, qui quadrado, teste t, correlação e descritivas. Os resultados apontaram prevalência de 31,3% para comportamentos autolesivos. Adolescentes do sexo feminino apresentaram 2,26 vezes mais chance de engajar na prática de comportamentos autolesivos. Verificou-se diferenças significativas entre participantes que se autolesionavam e aqueles que não se autolesionavam no tangente à percepção de sintomas depressivos, sintomas ansiosos e suporte familiar. Nossos achados enfatizam a importância dos vínculos familiares no combate aos comportamentos autolesivos. Assim, esperamos contribuir para a construção de estratégias preventivas contra comportamentos de risco, ansiedade e depressão.


Self-injurious behaviors in adolescence have become a public health problem worldwide. Often these behaviors are symptoms of psychiatric diseases, conceptualized as dysfunctional strategies of emotional regulation. Therefore, this study investigates the relation between self-injurious behaviors, family support perception and anxiety and depressive symptoms within adolescents from the state education network in Recife. It also estimates the prevalence of the phenomenon (n=501). Another goal was to estimate the prevalence of the phenomenon and its relation with sociodemographic variables. The instruments used were the Health Behavior in School-Aged Children Protocol, the Spence Children's Anxiety Scale and the Children's Depression Inventory, all adapted to Brazilian Portuguese. Internal consistency, chi-square, t-test, correlation and descriptive analyzes were performed. The results showed a prevalence of 31.3% for self-injurious behaviors, with female adolescents being 2.26 times more likely to engage in self-injurious behaviors. There were significant differences between participants who engaged in self-injurious behaviors and those who did not due to their perceptions of depressive symptoms, anxious symptoms and family support. Our findings emphasize the importance of family bonds in combating self-injurious behaviors. Thus, we hope to contribute to the creation of preventive strategies against risk behaviors, anxiety and depression.


Los comportamientos autolesivos en la adolescencia se han convertido en un problema de salud pública mundial. Estos comportamientos componen el cuadro sintomático de enfermedades psiquiátricas, siendo conceptualizados como estrategias disfuncionales de regulación emocional. Este estudio investiga la relación entre las conductas autolesivas, la percepción del apoyo familiar y los síntomas de ansiedad y depresión entre adolescentes de la red estatal de educación en Recife, además de estimar la prevalencia del fenómeno (n=501). También se buscó estimar la prevalencia del fenómeno y su relación con variables sociodemográficas. Los instrumentos utilizados fueron el protocolo Health Behaviour in School-Aged Children, la Spence Children's Anxiety Scale y el Invetario de Depresión Infantil, todos adaptados al portugués brasileño. Se realizaron análisis de consistencia interna, chi cuadrado, prueba t, correlación y descriptivos. Los resultados apuntaron para la prevalencia de 31,3% para comportamientos autolesivos. Adolescentes del sexo femenino presentaron 2,26 veces más chances de involucrarse en la práctica de comportamientos autolesivos. Se verificaron diferencias significativas entre participantes que se autolesionaban y aquellos que no se autolesionaban frente a la percepción de síntomas depresivos, síntomas ansiosos y apoyo familiar. Esperamos contribuir con la construcción de estrategias preventivas contra conductas de riesgo, ansiedad y depresión.

Background: Reports of suicidal behavior have increased in Mexico for years. In order to develop a more adequate suicide prevention strategy, it is necessary to understand its predictive factors, so the purpose of this research was to propose a model of suicidal risk in young people, taking into account one of the most current theories on the subject, Joiner's interpersonal theory. Method: A non-probabilistic sample of young people with suicidal ideation from three regions of Mexico was obtained by online survey (N=411), with mean age of 17.89 years (SD. 1.2), 336 women (81.8%), and 75 men (18.2%). Results: First, a multiple linear regression model was created to predict suicidal risk based on thwarted belongingness and perceived burden with 17% explained variance; then a second model was generated with the same variables and including other factors associated with suicide such as self-injury desires, impulsivity and suicide attempts, in addition to variables associated with family conflicts, improving the explained variance to 34%. Lastly, two properly adjusted structural equation models were obtained, one focused on suicidal risk (R2=.21; RMSEA=.026; CFI=.99) and the other on ideation (R2=.18; RMSEA=.070; CFI=.98). Conclusions: The main factors that explain suicidal risk are depressive symptoms, perceived burden and desires for self-injury. Further research on the effect of painful experiences as factors that could predict suicide attempt is suggested.


Introducción: Los reportes de conducta suicida en México han aumentado por años. Para desarrollar una estrategia más adecuada de prevención del suicidio es necesario comprender sus factores predictores, por lo que el propósito de la presente investigación fue proponer un modelo del riesgo suicida en jóvenes tomando en cuenta una de las teorías más vigentes en cuanto al tema, la teoría interpersonal de Joiner. Método: Se obtuvo por encuesta online una muestra no probabilística de jóvenes con ideación suicida de tres regiones de México (N=411), con una edad media de 17.89 años (DE. 1.2), 336 mujeres (81.8%) y 75 hombres (18.2%). Resultados: Primero se conformó un modelo de regresión lineal múltiple para predecir riesgo suicida a partir del sentido de pertenencia frustrado y la carga percibida con 17% de varianza explicada; después se generó un segundo modelo con las mismas variables e incluyendo otros factores asociados al suicidio como los deseos de autolesión, impulsividad e intentos suicidas, además de variables asociadas a conflictos familiares, mejorando la varianza explicada hasta un 34%. Por último, se obtuvieron dos modelos de ecuaciones estructurales con ajuste adecuado, uno enfocado en riesgo suicida (R2=.21; RMSEA=.026; CFI=.99) y otro en la ideación (R2=.18; RMSEA=.070; CFI=.98). Conclusiones: Los principales factores que explican el riesgo suicida son los síntomas depresivos, la carga percibida y los deseos de autolesión, se sugiere seguir investigando sobre el efecto de experiencias dolorosas como factores que podrían predecir el intento suicida.

The purpose of this paper is to review the research status on non-suicidal self-injury (NSSI) in children and adolescents, so as to provide references for the cognition and intervention of NSSI behavior in children and adolescents. NSSI behavior is an independent risk factor for suicide, which not only poses a serious threat to the mental health of children and adolescents, but also brings a great burden on families and society. This paper focuses on the epidemiology, etiology, relationship with other psychiatric disorders, and treatment of NSSI behaviors in children and adolescents.

ABSTRACT

ObjectiveTo establish a diagnostic prediction model for non-suicidal self-injury (NSSI) behaviors in adolescents with depressive disorder, in order to provide references for early identification of NSSI behaviors in them. MethodsRetrospective analysis was performed on the clinical data of adolescents with depressive disorder (n=366) who were admitted to the Pediatric Department of Shenzhen Kangning Hospital from January 1 to December 31, 2021. According to the Diagnostic criteria of Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) diagnostic criteria for NSSI, the patients were divided into comorbid NSSI group (n=289) and non-NSSI group (n=77). The selected adolescents were randomly divided into a training set (n=258) and a verification set (n=108) in a 7∶3 ratio. Logistic regression analysis was used to screen the independent risk factors for NSSI behaviors in adolescents with depressive disorder, which served as the basis for prediction model. Finally, the receiver operating characteristic (ROC) curve was established and the area under curve (AUC) was calculated to evaluate the discrimination in the training set and validation set. Calibration curve was applied to evaluate the calibration degree of the model. The Homser-Lemeshow (HL) test was conducted to evaluate the goodness of fit of the model. And decision curve analysis (DCA) was performed to evaluate the clinical benefit of the model. ResultsGender (β=1.734, OR=5.561, 95% CI: 2.678~11.964), education level (β=0.864, OR=2.737, 95% CI: 1.174~4.795), history of suicide attempts (β=0.932, OR=2.539, 95% CI: 1.253~5.144), being an only child (β=0.745, OR=2.106, 95% CI: 1.029~4.311) and depression severity (β=0.056, OR=1.058, 95% CI: 1.025~1.092) were independent risk factors related to NSSI behaviors in adolescents with depressive disorder (P<0.05 or 0.01). The AUC was 0.808 (95% CI: 0.746~0.870) in the training set, and was 0.722 (95% CI: 0.581~0.864) in the validation set. The prediction model showed good calibration with the HL test (P=0.561). ConclusionGender, education level, suicide attempt history, being an only child and depression severity are independent risk factors for NSSI behaviors in adolescents with depressive disorder, furthermore, the diagnostic clinical prediction model constructed using above factors for NSSI behaviors in adolescents with depressive disorder has displayed good sensitivity and specificity.

ABSTRACT

Non-suicidal self-injury (NSSI) is becoming increasingly common in adolescents and seriously affects their physical and mental health, and it is also a major risk factor for suicide among adolescents. NSSI has now become a public health issue of general concern; however, the identification of cognitive dysfunction in NSSI is still based on neuropsychological cognitive assessment and subjective questionnaire assessment, with a lack of objective evaluation indicators. As a method for studying the cognitive neural mechanism of NSSI, electroencephalography is a reliable tool for finding objective biomarkers of NSSI. This article reviews the recent research on electrophysiology associated with cognitive dysfunction in adolescents with NSSI.

ABSTRACT

Esta pesquisa teve como objetivo verificar a relação entre eventos traumáticos (ET) na infância e a ocorrência de comportamentos autolesivos em adolescentes. Os instrumentos utilizados foram o Questionário sobre Traumas na Infância (QUESI) e o Inventário de Autolesão Deliberada - reduzido (IAD-r). Participaram 494 estudantes do ensino médio de ambos os sexos e idade entre 15 e 18 anos (M = 16,4). Destes, 58,5% afirmaram ter sofrido abuso emocional de forma recorrente e 19,0% e 59,5% assumiram já ter sofrido abuso sexual e físico, respectivamente. Quanto à prática de autolesão, 65,0% revelaram já ter se engajado em comportamentos autolesivos. De acordo com a análise de Regressão Logística Binomial, todos os tipos de ET exibiram associação significativa com a prática de comportamentos autolesivos. A análise de moderação a respeito da interação entre a ocorrência de ET infantis e a prática de autolesão revelou ausência de moderação pelo sexo e pela idade. Porém, quanto ao abuso físico, o efeito de moderação da idade apresentou significância estatística limítrofe e indicou que os adolescentes mais novos, de 15 e 16 anos, que sofreram este tipo de abuso na infância, foram mais susceptíveis à prática autolesiva. Portanto, as altas taxas de ET e de autolesão encontradas nesta pesquisa revelam a gravidade do problema. Espera-se que esta investigação possa contribuir para a elaboração de intervenções para prevenção e controle dos fatores de risco que acometem a população infanto-juvenil.(AU)


This research aimed to verify the relationship between traumatic events (TE) in childhood and the occurrence of self-injurious behavior in adolescents. The instruments used were the Childhood Trauma Questionnaire (QUESI) and the Deliberate Self-Injury Inventory - reduced (IAD-r). The sample was composed of 494 high school students of both genders and aged between 15 and 18 years old (M = 16.4). Of those, 58.5% declared to have suffered recurring emotional abuse and 19.0% declared to have suffered sexual abuse and 59.5% physical abuse. Regarding the practice of self-harm, 65.0% reported having already engaged in self-injurious behaviors. According to the Binomial Logistic Regression analysis, all types of TE were associated with the practice of self-injurious behaviors. The moderation analysis between the occurrence of childhood TE and self-injury showed no moderation by sex or age. However, regarding physical abuse, the moderating effect of age showed borderline statistical significance and indicated that younger adolescents, 15 and 16 years old, who suffered this type of abuse in childhood, were more susceptible to self-injurious behavior. Therefore, the high rates of TE and self-injury found in this research reveal the seriousness of the problem. It is hoped for this investigation to contribute to the development of interventions to prevent and control risk factors that affect children and adolescents.(AU)


Esta investigación tuvo como objetivo verificar la relación entre eventos traumáticos (ET) en la infancia y la ocurrencia de conductas autolesivas en adolescentes. Los instrumentos utilizados fueron el Cuestionario de Trauma Infantil (QUESI) y el Inventario de Autolesiones Deliberadas -reducido (IAD-r). Participaron 494 estudiantes de la secundaria, de ambos sexos y con edades entre 15 y 18 años (M = 16,4). De estos, el 58,5% afirmaron haber sufrido maltrato emocional de forma recurrente, el 19,0% dijeron haber sufrido maltrato sexual y el 59,5%, maltrato físico. En cuanto a la práctica de la autolesión, el 65,0% informaron haber realizado conductas autolesivas. El análisis de Regresión Logística Binomial mostró que todos los tipos de ET tuvieron una asociación significativa con la práctica de conductas autolesivas. El análisis de la moderación respecto a la interacción entre la ocurrencia de ET infantil y la práctica de la autolesión reveló una ausencia de moderación por sexo o edad. En cuanto al maltrato físico, el efecto moderador de la edad mostró una significación estadística marginal e indicó que los adolescentes más jóvenes, de 15 y 16 años, que sufrieron este tipo de maltrato en la infancia, son más susceptibles a la práctica de autolesiones. Por lo tanto, las altas tasas de ET y autolesiones encontradas en esta investigación revelan la gravedad del problema. Se espera que esta investigación contribuya con el desarrollo de intervenciones para la prevención y control de los factores de riesgo que afectan a niños y adolescentes.(AU)

ABSTRACT

Algunas indagaciones sobre el suicidio indican que en muchos países se notifican las tasas más altas a escala mundial. Este se ubica entre las principales causas de muerte en adolescentes, jóvenes y en la población adulta y económicamente activa. El propósito del presente artículo es realizar un análisis crítico y reflexivo acerca de la importancia del cuidado de enfermería integral a la persona con intento suicida. Es imperativo concientizar en todos los ámbitos, la necesidad de prevenir el suicidio y que las personas puedan encontrar la ayuda en momentos de ideaciones suicidas sin temor a ser juzgados o señalados. En tal sentido, el cuidado que proporcionan los profesionales de enfermería en todas sus concepciones filosóficas resulta fundamental en la práctica diaria para abordar de manera integral al paciente con intento autolítico.


Research indicates that many countries report high rates of suicide. Suicide is among the main causes of death in adolescents, young adults and generally in the adult and economically active population. The purpose of this article is to carry out a critical and reflective analysis about the importance of comprehensive nursing care for the person who has attempted suicide. It is imperative to raise awareness of this problem and prevent suicide attempts by helping people find help in times of suicidal ideation without fear of being judged or singled out. The philosophical orientation and care provided by nursing professionals is essential in daily practice to comprehensively address the patient with the intent of self-harm.

As lesões autoprovocadas são investigadas a partir da escuta psicanalítica de 15 adolescentes que chegaram a um ambulatório público. O sofrimento psíquico na adolescência contemporânea apresenta-se com crescente frequência sob a forma desse tipo de problema. A medicalização, que tem sido uma resposta dominante para esses jovens, é avaliada criticamente. Explora-se, com a ajuda do referencial psicanalítico, outros tipos de entendimento da questão, recorrendo-se às teorias freudianas sobre angústia, dor e formação de sintomas. Usando os aportes lacanianos, são destacados, também, os efeitos da palavra e da voz do Outro sobre o sujeito adolescente. Dois jovens transgêneros que apresentavam histórico de autoagressão conduziram a reflexões específicas sobre o tema.


Self-inflicted lesions are examined from the perspective of psychoanalytic listening to fifteen adolescents who sought help at a walk-in clinic. Psychological suffering among contemporary adolescents often presents in the guise of this problem. Medicalization is critically assessed as the predominant response to such youths. Aided by psychoanalytic references, other modes of understanding the problem are explored, and Freudian theories of anxiety, pain, and symptom formation are deployed. Additionally, Lacanian theory is called upon to highlight the effects of the word and voice of the Other upon the adolescent subject. Two transgender youths presenting with histories of self-aggression lead to specific reflections on the issue.


Les automutilations sont étudiées à partir de l'écoute psychanalytique de 15 adolescents arrivés en centre ambulatoire. La souffrance psychique de l'adolescence contemporaine se manifeste de plus en plus fréquemment sous cette forme. La médicalisation, qui a été la solution majoritairement adoptée pour ces jeunes, est considérée sous un jour critique. Sont explorées, à l'aide du référentiel psychanalytique, d'autres formes de compréhension de cette problématique, grâce aux théories freudiennes sur l'angoisse, la douleur et la formation des symptômes. Faisant usage des apports lacaniens, sont également mis en relief les effets de la parole et de la voix de l'Autre sur le sujet adolescent. Deux jeunes transgenres présentant un historique d'automutilations ont conduit à des réflexions spécifiques sur le sujet.


Se investigan las autolesiones a partir de la escucha psicoanalítica de 15 jóvenes que fueron acudidos en ambulatorio público. El sufrimiento psíquico en la adolescencia contemporánea se presenta cada vez con mayor frecuencia en forma de este tipo de problemas. La medicalización, que ha sido una respuesta dominante para estos jóvenes, es evaluada críticamente. Con la ayuda del marco psicoanalítico, se exploran otras maneras de comprensión del problema, utilizando las teorías freudianas sobre la angustia, el dolor y la formación de síntomas. Con los aportes lacanianos, también se destacan los efectos de la palabra y la voz del Otro en el sujeto adolescente. Dos jóvenes transgénero que tenían un historial de autolesiones llevaron a reflexiones específicas sobre el tema.

Resumo A autolesão entre mulheres jovens é problema de saúde pública ainda pouco conhecido e compreendido por pais, educadores e profissionais de saúde. Procurou-se compreender o comportamento autolesivo em mulheres jovens numa perspectiva do significado, ações e interpretação da situação vivenciada. Trata-se de pesquisa com abordagem qualitativa que utilizou entrevista semiestruturada para coleta de dados, no período de janeiro a março de 2020. Os dados foram organizados pelo programa MAXQDA e analisados com base no Interacionismo Simbólico. As cinco entrevistadas são jovens educadas pelas mães, possuem pouco ou nenhum contato com os pais. Narraram histórias de abuso sexual, rejeição paterna, bullying e baixo acolhimento no ambiente escolar. Estabeleceram uma percepção pessimista de si, oriunda de interpretações próprias e de suas interações sociais. Enxergaram a autolesão como refúgio. Praticaram a autolesão quando estavam sob sentimentos negativos insuportáveis. Viviam num ciclo de substituição do sofrer psicológico pelo padecimento físico. Todas admitiram possuir temperamentos ansiosos, baixa autoestima e inabilidades socioemocionais. A autolesão tem vínculo direto com os significados que essas jovens se atribuem. Nas escolas, a incorporação de conhecimento sobre bem-estar deve ser estimulada para a formação de pessoas mais eficazes na resolução de problemas.


Abstract Self-injury among young women is a public health problem that is still little known and understood by parents, educators and health professionals. We sought to understand selfinjurious behavior in young women from a perspective of the meaning, actions and interpretation of the experienced situation. This qualitative research used semi-structured interview for data collection from January to March 2020. The data were organized by the MAXQDA software and analyzed based on Symbolic Interactionism. The five interviewees are young people educated by their mothers, with little or no contact with their fathers. They narrated stories of sexual abuse, parental rejection, bullying and low acceptance in the school environment. They established a pessimistic perception of themselves arising from their own interpretations and their social interactions. They saw self-injury as a refuge. They practiced self-injury when they were under unbearable negative feelings. They lived in a cycle of substituting psychological suffering for physical suffering. All admitted having anxious temperaments, low self-esteem and socioemotional disabilities. Self-injury is directly linked to the meanings these young women give to themselves. At schools, the incorporation of knowledge about well-being should be encouraged to train people who are more effective in solving problems.

ABSTRACT

ObjectiveTo explore the characteristics of adolescent non-suicidal self-injury (NSSI) behavior, and to enable clinicians to further understand adolescents with NSSI behavior, so as to make better clinical diagnosis and intervention. MethodsFrom July 2022 to June 2023, 120 adolescent patients with NSSI behavior were selected from the outpatient department of our hospital by convenience sampling, and the general demographic data were collected by self-made general situation questionnaire. The characteristics and motivation of NSSI among adolescents with NSSI were analyzed by using the NSSI questionnaire and Ottawa self-injury inventory (OSI). ResultsThe average age of the first NSSI in the adolescents was 12.90±1.233, and the most common was 13 years old. The most common site of NSSI was the lower arm or wrist (58.33%), followed by the hand (27.5%). The most commonly used NSSI was "intentionally cutting oneself" (68.33%); The most common motivation for NSSI is emotional regulation. ConclusionThere are significant differences in gender among adolescents with NSSI behavior. The age of first NSSI is concentrated, and the highest incidence is in the 11‒14 years old. The common way of self-injury is cutting. NSSI is usually associated with the intention to relieve suffering and is characterized by high frequency, using variety of methods, and low mortality. Adolescents with NSSI often choose to implement NSSI with emotion regulation as the main function, including external emotion regulation and internal emotion regulation.

ABSTRACT

BackgroundNon-suicidal self-injury (NSSI) is a serious global public health issue and an important risk factor for suicide attempts and completed suicide. The incidence of NSSI among adolescents in China is 27.4%. Therefore, it is critical to address NSSI and prevent its progression into more severe mental health conditions. ObjectiveTo systematically evaluate the psychological experiences and needs of adolescents with NSSI behavior, so as to provide references for formulating targeted intervention strategies for this demographic. MethodsA computer search was conducted across a total of eleven databases, including Cochrane Library, Embase, PubMed, PsycINFO, Web of Science, CINAHL, Medline, CNKI, Wanfang, VIP and CBM, to gather qualitative research on the psychological experiences and needs of adolescents with NSSI behavior. The search extended from the establishment of these databases up to March, 2023. The Australian Joanna Briggs Institute (JBI) qualitative research quality evaluation tools were used to evaluate the included literature, and a Meta-synthesis method was used to integrate the results. ResultsA total of 11 studies were included, yielding 46 research findings. Similar findings were consolidated into 10 new categories, ultimately resulting in 5 synthesized outcomes, including interpersonal conflict, academic pressure and self-reflective struggles, self-injurious behaviors intertwined with inner emotions, the power of love to overcome helplessness, and adjustment and seeking positive coping strategies. ConclusionAdolescents often have experienced negative life events before NSSI behaviors, with notable conflicts in interpersonal relationships, academic pressures and family education. Following NSSI behavior, they undergo various psychological experiences, such as transient feelings of relief, dependence and guilt. ABSTRACT

BackgroundThe major depressive disorder has high prevalence among adolescents, and non-suicidal self-injury (NSSI) behaviors frequently occur among patients, therefore, major depressive disorder in adolescents has become the researching focus. ObjectiveTo explore the effect of mentalization-based family therapy (MBFT) on depressive symptoms and NSSI behavior in adolescents with major depressive disorder, and to provide references for the rehabilitation of major depressive disorder in adolescents. MethodsA total of 90 adolescent patients with major depression disorder who met the diagnostic criteria of International Classification of Diseases, 10th edition (ICD-10) for depressive disorders and attended Wuhan Mental Health Center from January to December 2022 were selected, and were assigned into study group (n=44) and control group (n=46) using random number table method. All participants received routine intervention, based on this, study group added a 60-minute MBFT intervention once a week for 8 weeks. Before the intervention and at the end of 1st, 2nd,4th and 8th week,the two groups were assessed using Hamilton Depression Scale-24 item (HAMD-24), General Self-Efficacy Scale (GSES), Pittsburgh Sleep Quality Index (PSQI) and Ottawa Self-injury Inventory (OSI). ResultsThe repeated measures analysis of variance reported a statistical main effect of time, main effect of group, and interaction effect between time and group at the baseline and the end of 1st, 2nd, 4th and 8th week of treatment in HAMD-24 score (F=69.621, 15.428, 29.623, P˂0.05), OSI score (F=176.642, 37.682, 21.873, P˂0.05), GSES score (F=215.236, 57.421, 27.857, P˂0.05) and PSQI score (F=268.541, 61.863, 33.867, P˂0.05). Individual effect analysis discovered a statistical difference between study group and control group at the end of 2nd, 4th and 8th week of treatment in HAMD-24 score (t=5.567, 8.645, 6.233, P˂0.01), OSI score (t=3.675, 11.817, 9.632, P˂0.01), GSES score (t=23.462, 31.709, 12.750, P˂0.01) and PSQI score (t=9.664, 22.457, 9.333, P˂0.01). ConclusionMBFT may improve depressive symptoms, NSSI behavior, sleep quality and self-efficacy in adolescents with major depressive disorder. ABSTRACT

BackgroundNon-suicidal self-injury (NSSI) behavior poses a significant risk to the psychosomatic health of middle school students. However, the intrinsic mechanisms underlying the risk factors associated with NSSI behavior remain unclear. ObjectiveTo explore the relationship and underlying mechanism between depressive mood, impulsivity, and NSSI behavior in middle school students, and provide valuable insights for the prevention and intervention of NSSI behavior in this population. MethodsIn March 2021, 1 161 middle school students were recruited by cluster random sampling. They were assessed using Adolescent Non-Suicidal Self-Injury Assessment Questionnaire (ANSAQ), Center for Epidemiologic Studies Depression Scale (CES-D), and Barratt Impulsivity Scale (BIS-II). Spearman's correlation analysis was used to examine the correlation between depressive mood, impulsivity and NSSI behavior in middle school students. Moreover, the Process mediation effect analysis was adopted to test the mediating role of impulsivity between depressive mood and NSSI behavior. ResultsTotal of 122 people(10.51%) with NSSI behavior were detected. CES-D score was positively correlated with the total score of BIS-II and scores of all dimensions (r=0.340~0.556, P<0.01), and also were positively correlated with the total score of ANSAQ and scores of all dimensions (r=0.437~0.540, P<0.01). Additionally, the total score and dimension scores of BIS-II were positively correlated with the total score and dimension scores of ANSAQ (r=0.157~0.437, P<0.01). Impulsivity played a partial mediating role between depressive mood and NSSI behavior [indirect effect=0.035 (95% CI: 0.004~0.067, P<0.01), indirect effect accounted for 5.81% of the total effect]. ConclusionDepressive mood in middle school students has both a direct and indirect influence on NSSI behavior, partially mediated by impulsivity. ABSTRACT

Objective:To investigate the correlation between cognitive function and addiction, impulsivity, and anhedonia in adolescent depressive disorder patients with self-injury behavior.Methods:From September 2021 to October 2022, adolescents with depressive disorders who visited the outpatient department of the Qingdao Mental Health Center were enrolled and divided into self-injury group and non self-injury group based on the presence or absence of self-injury behaviors, each with 60 participants.A self-compiled general information questionnaire, the 17 items Hamilton depression rating scale (HAMD-17), the Ottawa self-injury inventory (OSI), the Chinese version of the Barratt impulsiveness scale (BIS-11), and the temporal experience of pleasure scale(TEPS) were used to evaluate both groups.The Chinese brief cognitive test(C-BCT) was used to assess cognitive function in both groups.SPSS 26.0 software was used for statistical analysis, including t-test, χ2 test, Pearson correlation analysis, and multiple linear regression analysis. Results:The self-injury group had higher scores for OSI addiction factors (9.43±8.29) and BIS-11 (67.09±11.48) compared to the non self-injury group (OSI addiction factor scores: 0, BIS-11 scores: 53.70±7.12, t=6.22, 5.91, both P<0.05). TEPS score and C-BCT scores in various dimensions were lower in the self-injury group than those in the non self-injury group ( t=-2.93, -2.01, -2.88, -2.20, -5.35, all P<0.05). Information processing speed was negatively correlated with BIS-11 score ( r=-0.296, P<0.05), and attention score were negatively correlated with OSI addiction factor score and BIS-11 score ( r=-0.303, -0.561, both P<0.05) and positively correlated with TEPS score ( r=0.364, P<0.05), including a positive correlation with the scale of anticipatory anhedonia score ( r=0.318, P<0.05). Working memory score was negatively correlated with OSI addiction factor score and BIS-11 score ( r=-0.312, -0.416, both P<0.05). Comprehensive ability and executive function scores were negatively correlated with OSI addiction factor score and BIS-11 score ( r=-0.308, -0.679, both P<0.05), and positively correlated with TEPS score ( r=0.304, P<0.05). Multiple linear regression analysis showed that BIS-11 scores were influencing factors of C-BCT dimensions ( β=-0.260, -0.592, -0.557, -1.797, t=-2.150, -3.314, -2.285, -5.165, all P<0.05). Conclusion:In adolescent depressive patients with self-injury, cognitive function is correlated with addiction, impulsivity and anhedonia, among which impulsivity is a risk factor for cognitive function.
